All lunches are subject to payment. The price for the first meal served is set by your free and reduced meal application. Additional lunches are available at the non-reimbursable price, $5.00. Submit bottom portion of this form WITH PAYMENT ONLY to the office in the envelope provided. If NOT ordering lunch, please write ‘0’ in the number of lunches column.
